Our mission is to enhance the quality of life for adults with disabilities or illness through a medical, supportive and rehabilitation day program, which maximizes independence and enables the individual to remain in his/her environment.

Our services include assessments and care planning, health-related and social services, therapeutic activities, nutrition, and transportation, and are consistent with the economic, cultural and social needs of the community. They are focused on the participant's strengths and abilities.

Our team is able to create a physical environment that will provide safety and structure for participants. We also establish relationships and create a culture that supports, involves, and validates the participants.

Our Servies

Millennium Home ADHC, our Registered and Licensed Practical Nurses have the qualification to deliver a comprehensive range of nursing services tailored to your specific needs and under the guidance of your physicians. Our main priority is preventive healthcare, aiming to identify and address any emerging symptoms or medication-related issues promptly. We collaborate closely with your Primary Care Provider to ensure seamless coordination among all members of your care team, fostering your ability to thrive safely within the community.
• Individually Observation and complete care assessments: Tailored assessments designed specifically for each participant to monitor their health status and provide personalized care plans.
• Wound care: Specialized care for chronic, non-healing wounds caused by conditions such as diabetes, circulatory deficiencies, pressure ulcers, and post-surgical complications.
• Medication management and teaching: Assistance with medication administration and education on proper usage. Includes bowel/bladder training to help manage continence.
• Diabetic care and Blood Sugar monitoring education: Guidance and education on managing diabetes, including daily blood sugar monitoring and insulin administration.
• Foley catheter care: Insertion and teaching of care for indwelling Foley catheters to ensure proper hygiene and prevent complications.
• Administration of IM/Subq. medications: Administration of intramuscular or subcutaneous medications such as vitamin B12 or insulin.
• Teaching ostomy or ileo conduit care: Education and support for individuals with ostomies or ileo conduits to manage their care effectively.
• Gastrostomy feeding and parenteral nutrition teaching: Instruction on feeding methods for individuals with gastrostomy tubes and education on parenteral nutrition when necessary.
• Inhalation Rx administration: Administration of prescribed inhalation medications to manage respiratory conditions.
• Daily monitoring of vital signs: Regular monitoring of vital signs such as blood pressure, heart rate, temperature, and oxygen saturation to track health status and detect changes.
• Personal care assistance: Help with activities of daily living including showering, dressing, toileting, transferring, ambulation, eating and other personal care needs to support independence and well-being

At Millennium Home ADHC, we provide tailored recreational and therapeutic programs aimed at enhancing your independence and overall well-being. These activities are carefully crafted to suit your unique needs, offering cognitive stimulation, memory enhancement, physical fitness promotion, and opportunities for self-expression.
Our professional teams collaborate with you to design therapeutic activities that align with your preferences and requirements. We focus on improving your physical strength, cognitive abilities, and self-reliance. We value your input and regularly convene Participant Advisory Councils to gather feedback on our scheduled activities such as:
- daily morning exercises, yoga, Zumba, dance hours and YMCA field trips.
- ESL, craft and music classes.
- trips to local theaters, restaurants, museums and parks and many others.

We provide a supportive and stimulating environment for adults who require supervision, assistance with activities of daily living, or medical care during the day. We offer such facility services as:
1. Transportation: We offer transportation services to help participants travel to and from the facility. Our trained drivers assist participants with mobility needs.
2. Nutritious Meals: We provide participants with 2 hot meals and snacks during their stay. These meals are designed to meet dietary needs and preferences, taking into account any specific health conditions or dietary restrictions.
3. Social Services: Our trained staff assist our participants with their Social work needs interpreting their mail or other correspondence from health care and government agencies and also coordinating them with our nursing staff, therapeutic activities staff, and alternative therapists.
4. Rehabilitation Services: We provide services from an acupuncturist, podiatrist, and audiologist.
5. Therapeutic Activities: We offer different recreational and therapeutic activities tailored to our participants' individual needs and designed to help improve and maintain their level of independence and wellness.
These activities are cognitively stimulating, help to enhance their memory and promote physical fitness, and create opportunities for unique self-expression.
Millennium Home offers both skilled and nonskilled services, in addition to the variety already provided, to meet the needs of all participants.

The Physical Therapist offers a full range of services such as gait training, ultrasound therapy, assistance with transferring, home exercise programs, prosthetic training, and instruction on using medical equipment.
The Occupational Therapist conducts evaluations and provides training in activities of daily living, muscle re-education, fine motor coordination, perceptual motor skills, neuro-developmental techniques, sensory treatments, orthotics and splinting.
The Speech Therapist specializes in treating voice disorders, speech articulation difficulties, swallowing disorders, language impairments, and oral rehabilitation.
The Respiratory Therapist delivers targeted treatments.
Our Registered Dietitian offers guidance on adopting healthy eating habits and maintaining a balanced diet for optimal health.

Activities: Our tailored activity program is designed specifically for individuals with Alzheimer's and dementia. These activities not only stimulate normal brain function but also provide enjoyment. Daily activities may include music, art, recreation and discussion groups, and support sessions.
Counseling: Our center offers supportive services for individuals with dementia and their families. This may involve offering advice on accessing external resources and arranging additional care services at home.
Specialized Support: We are equipped to cater to participants with various special needs, including wheelchair accessibility, hearing or vision impairment, incontinence, hallucinations, or speech difficulties.
